A Regional Collaboration for Health (ARCH Home (arch.wales) ) is a unique collaboration between three strategic partners; Swansea Bay University Health Board, Hywel Dda University Health Board and Swansea University. It spans the local authority areas of Ceredigion, Pembrokeshire, Carmarthenshire, Neath Port Talbot and Swansea. ARCH have been tasked to complete a Population Health Needs Assessment and this survey has been created to provide you, the public, with the opportunity to have your say and to let the Health Boards of Hywel Dda and Swansea Bay know what we can do to support you with your health and wellbeing needs. The information is anonymous, but please complete all fields that are relevant to you to the best of your ability, as to provide us with as much information as possible to support us in planning our future, based on what you report are your identified needs. We would also like your support in assisting anyone that you care for that receive or access any of the Health Boards services to complete the form. Y mae Cydweithrediad Rhanbarthol ar gyfer Iechyd (ARCH ARCH Home ) yn gydweithrediad unigryw rhwng tri phartner strategol; Bwrdd Iechyd Prifysgol Bae Abertawe, Bwrdd Iechyd Hywel Dda a Phrifysgol Abertawe.Mae'n cwmpasu ardaloedd awdurdodau lleol Ceredigion, Sir Benfro, Sir Gaerfyrddin, Castell-nedd Port Talbot ac Abertawe. Mae ARCH wedi cael eu gofyn i gwbwlhau Asesiad o Anghenion Iechyd y Boblogaeth ac y mae’r arolwg hwn wedi’i greu i roi’r cyfle i chi, y cyhoedd, i ddweud eich dweud ac i roi gwybod i Fyrddau Iechyd Hywel Dda a Bae Abertawe beth allwn ni ei wneud i’ch cefnogi gyda’ch anghenion iechyd a lles. Mae’r wybodaeth yn ddienw, ond cwblhewch bob maes sy’n berthnasol i chi hyd eithaf eich gallu, er mwyn rhoi cymaint o wybodaeth â phosibl i ni i’n cefnogi wrth gynllunio ein dyfodol, yn seiliedig ar yr hyn yr ydych yn adrodd yw eich anghenion canfyddedig. Hoffem hefyd gael eich cefnogaeth i helpu unrhyw un yr ydych yn gofalu amdano sy'n derbyn neu'n defnyddio unrhyw un o wasanaethau'r Bwrdd Iechyd i lenwi ffurflen ar wahân.
